  • Names and telephone numbers of persons to contact in an emergency

We do not use government-related identifiers for identification purposes, although information in our records may include such identifiers if provided by you. In some cases, we may collect sensitive information in compliance with local data protection laws and with your consent. 


If you are applying for employment with us, personal information related to your application will also be collected.
